About Us

The mission and vision of Health Improvement Alliance (HIA) is to work together to identify, prioritize, and measurably improve the health of the community through prevention, education, and services.

Initially named the Primary Care Action Group (PCAG), the coalition changed names in 2019 to HIA to more accurately reflect the expanded focus of our work.

HIA is a coalition of two neighboring hospitals, Bridgeport Hospital and St. Vincent’s Medical Center, along with the seven departments of public health, federally qualified health centers, and about 85 community and non-profit organizations all serving the Greater Bridgeport Region.

Our Structure

Coordinating Team

Provides overall guidance and planning for HIA. This team was added in fall of 2023 as a result of feedback received from our partnership survey. The team is comprised of representatives from the Greater Bridgeport CHNA funding organizations and HIA task force co-chairs.

HIA Community Coalition

This is the monthly HIA general meeting. All are invited to attend. Topics will vary by month. This meeting is a great opportunity for networking, information sharing, presentations/education, and sharing of best practices.

HIA Workgroups

Where most of the work of HIA happens. Previously known as task forces, HIA has begun a restructuring of workgroups alongside the release of the 2025 Greater Bridgeport CHNA. In years prior, each task force focused on a single priority area identified through the regional CHNA process and had their work guided by the Community Health Improvement Plan (CHIP).

While the CHNA and CHIP still drive the work, we have moved from a needs-based approach to a strategy-based approach, where each workgroup will address all selected priority areas, to reduce operating in silos. The workgroups are as follows: Education and Training Workgroup; Outreach and Screening Workgroup; and Resource Development and Dissemination Workgroup. Membership includes representatives from local organizations and communities from throughout the Greater Bridgeport region. Workgroup membership is open to anyone that lives/works in the region and is interested in getting involved.

Education and Training Workgroup

All CHNA priority areas will be addressed, with the work focused on the strategies of education and training for both the community and HIA members. Examples include but are not limited to: Documentary Film Screenings, Mental Health First Aid trainings, and Cultural Awareness trainings.

Outreach and Screening Workgroup

All CHNA priorities will be addressed, with the work focused on the strategies of community outreach and health and wellbeing screening activities. Examples include but are not limited to: Know Your Numbers screenings (blood pressure, blood sugar, body mass index, with health education), Health Fairs, and community events/forums.

Resource Development and Dissemination Workgroup

All CHNA priorities will be addressed, with the work focused on creating, improving, and distributing resources for the community and local professionals. Examples include but are not limited to: HIA Interactive Linktree, Primary Care Providers List, Food Resource Guides, and Immunization Guides.
