HIA and its partners are committed to keeping our community informed through transparent, data-driven reporting. In this section, you will find the core documents that guide our work to improve the health and wellbeing of Greater Bridgeport residents.

Community Health Needs Assessment (CHNA)

The Community Health Needs Assessment is a comprehensive, data-based report produced every three years in partnership with local hospitals, public health agencies, and community organizations. The CHNA identifies the most pressing health and social needs in our region by analyzing local data and listening to input from residents and community partners.

Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan is the action-oriented companion to the CHNA. Based on the needs identified in the CHNA, the CHIP outlines shared goals, strategies, and measurable objectives that HIA partners work on together over a three-year period. It serves as a roadmap for collaboration designed to strengthen community health and address the social conditions that influence wellbeing. HIA Annual Report

Released every October, the HIA Annual Report highlights the progress being made across the coalition in advancing our priorities identified in the CHNA and CHIP. This report celebrates the achievements of our partners, showcases data and stories from throughout the year, and demonstrates the collective impact of the Greater Bridgeport health ecosystem in responding to community health and social needs.
